The Rapid Response Social Workers are co-located with the enhanced care teams in one of five geographical areas across the county but will provide a cross county service out of core hours.

The main purpose of the team is to provide residents of Worcestershire with health, social care and promoting independence services as near to home as possible, to ensure a rapid response to problem solving when adults find themselves in a crisis situation which might lead to an inappropriate admission to hospital.

It is essential that a social worker has substantial knowledge of the assessment of needs for adult service users, knowledge of long term conditions, the ability to work with adults, primarily older people and possibly young adults with physical disabilities, knowledge and understanding of health and social care systems and experience of multi-agency working.

Candidates must have:

HCPC Registration

CV

Enhanced DBS within the last 12 months

5 Years References

Knowledge of the Mental Health Act 1983

Professional qualification in Social Work (Dip SW, CQSW, CSS or social work degree)

Current full driving license
